Registration for this screening will open to the public in a few weeks, please fill out the form below to be notified.

NOTICE: If you are seeking a screening because you child is complaining of a concerning symptom such as chest pain, shortness of breath, dizziness, or palpitations, follow up immediately with your child’s physician. Student Heart Check is not intended as a supplement to immediate medical care in the case of any heart related symptoms, or in replacement of a cardiology appointment or heart tests your physician recommends your student have. In those cases, physicians should write a referral to cardiology.

Student Heart Check screenings are for ages 13-18. Please fill out the form to be notified of when screening registration is open to the public. We will not advertise the screening registration for each event until the host school has time to advertise to their own students for priority registration.

Notification from this list does not guarantee an appointment at the screening. We will provide you with a link to register your student, which fill quickly. You will receive an email for the upcoming screening only.
