Fifty-two Beaumont Medical Group practices have earned patient-centered medical home designation from Blue Cross Blue Shield of Michigan. These Beaumont Medical Group practices were nominated by their physician organizations or accountable care organization based on demonstrated evidence of required capabilities, quality and utilization scores.

The physician organizations representing Beaumont Health physicians include: United Physicians for Beaumont Medical Group; Oakwood Accountable Care Organization for Oakwood Physicians (OPi); and Olympia Medical Services and Oakland Southfield Physicians for Botsford Clinic System.

“The PCMH designation transforms health care to a patient-centered, efficient, cost-effective care model. It changes the way our physicians take care of patients, run their offices and interact with specialists and the hospital,” says Terry Kowalenko, M.D., president, Beaumont Medical Group. “This is especially important as part our new organization’s focus, is to provide exceptional patient and family centered care, as well as provide value- based care.”

Patient-centered medical homes are an approach to providing comprehensive primary care that facilitates partnerships between individual patients and their health care providers, and when appropriate, the patient’s family. The goal is to allow better access to health care, increase satisfaction with care and improve health.

The transition to patient-centered medical homes has been strongly endorsed and encouraged nationwide by payers such as Blue Cross and the Centers for Medicare & Medicaid Services. In Michigan, Blue Cross Blue Shield offers financial incentives for primary care physicians to support efforts to move away from the traditional fee-for-service practice model and toward a “value-based” reimbursement model, collaborating directly with physicians through their physician organizations. Primary care practices that demonstrate a commitment to value-based care by becoming patient-centered medical homes qualify for these financial incentives.

A recent analysis of Blue Cross claims data shows that PCMH-designated doctors are succeeding in more effectively managing patients’ care to keep them healthy and prevent complications.
